Medical Social Worker, Home Health Visits

📋 Description

• Assist team in understanding social and emotional factors affecting clients’ health.
• Provide clinical social evaluations and plan interventions based on findings.
• Identify community resources to meet client and family needs.
• Advocate for the client and family to navigate community services.
• Participate in developing the total care plan with the multidisciplinary team.
• Develop and maintain contacts with public/private agencies as resources.

🎯 Requirements

• Master’s degree in social work with at least 2 years of community-based experience.
• CSWE accreditation demonstrated by transcript or degree.
• Home health care experience is a plus.
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.
