Medical Social Worker, Home Health Visits

📋 Description

• Making home visits to clients in designated territories.
• Assess social and emotional factors related to clients' health problems.
• Provide clinical social evaluations and plan interventions based on evaluation findings.
• Identify community resource referrals for client and family needs.
• Advocate for client and family to navigate the community system.
• Participate in the development of the total plan of care with the multidisciplinary BAYADA clinicians.

🎯 Requirements

• Licensed by NJ Board of Social Work Examiners at LMSW or LISW.
• MSW with 2+ years of community-based experience.
• CSWE accreditation demonstrated by transcript or degree.
• Home care experience a plus.
• English proficiency (read, write, communicate).
• Ability to travel to cases.
