Medical Social Worker, Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Assess social and emotional factors affecting health.
• Provide clinical social evaluations and plan interventions.
• Identify community resource referrals to address client and family needs.
• Advocate for client and family through the community system.
• Document observations and interventions using EMR on a tablet.

🎯 Requirements

• Must be licensed by Maryland Board of Social Work Examiners at LMSW or LISW.
• MSW degree with at least 2 years of community-based experience.
• CSWE accreditation demonstrated by transcript or degree.
• Home care experience is a plus.
• Ability to read, write, and communicate in English.
• Ability to travel to cases as assigned.
