Medical Social Worker - Home Health Visits

📋 Description

• Making home visits to clients in designated geographic territories.
• Assess social/emotional factors related to health problems.
• Provide clinical social evaluations and plan appropriate interventions.
• Identify appropriate community resource referrals to address the client’s and the family’s practical and environmental needs.
• Advocate for both the client and the family to navigate the patient through the community system.
• Document observations, interventions, and evaluations pertaining to patient care management and services provided utilizing a state-of-the-art touch pad tablet.

🎯 Requirements

• Must be licensed by the Maryland Board of Social Work Examiners at the LMSW or LISW level.
• A Master's in Social Work with at least 2 years of community-based experience.
• Accredited by CSWE (demonstrated by transcript or degree).
• Home care experience a plus.
• Ability to read, write, and communicate in English; strong interpersonal skills.
• Ability to travel to cases; solid computer skills; EMR experience preferred.
