Medical Social Worker - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Assess social and emotional factors related to clients' health problems.
• Provide clinical social evaluations and plan appropriate interventions.
• Identify community resource referrals to address client and family needs.
• Advocate for clients and families to navigate the community system.
• Develop and maintain the total plan of care with the multidisciplinary BAYADA team.

🎯 Requirements

• Must be licensed by the Maryland Board of Social Work Examiners at the LMSW or LISW level.
• Master of Social Work (MSW) with at least 2 years of community-based experience.
• Accreditation by CSWE as demonstrated by transcript or degree.
• Solid computer skills; EMR experience preferred.
• Ability to travel to assigned cases.
• Read, write, and communicate effectively in English.
