Medical Social Worker, Home Health Visits

📋 Description

• Assist team in social and emotional factors affecting clients' health.
• Provide clinical social evaluations and plan interventions.
• Identify community resources to address client and family needs.
• Advocate for client and family to navigate the community system.
• Contribute to the total plan of care with BAYADA clinicians.
• Develop and maintain contacts with public/private agencies for resources.
• Educate and instruct patients and families on community resources for health and independence.
• Accurately document observations and interventions using a touchpad tablet.

🎯 Requirements

• MSW with 2+ years of community-based experience
• CSWE accreditation evidenced by transcript or degree
• Home health care experience preferred
• Solid computer skills; EMR experience preferred
• Ability to travel to cases as assigned
