Provide clinical social evaluations and plan interventions based on evaluations.
Identify community resource referrals to address practical and environmental needs.
Advocate for clients and families to navigate the community system.
Document observations and interventions using a tablet EMR.

🎯 Requirements

Licensed by NJ Board of Social Work Examiners at LMSW or LISW.
Master's in Social Work with at least 2 years of community-based experience.
CSWE accreditation demonstrated by transcript or degree.
Home care experience a plus.
Ability to read, write, and communicate in English.
Ability to travel to cases.
Solid computer skills; EMR experience preferred.
