Medical Social Worker, Home Health Visits

📋 Description

• Making home visits to clients in designated geographic territories.
• Assess social and emotional factors related to clients’ health problems.
• Provide clinical social evaluations and plan appropriate interventions based on evaluation findings.
• Identify community resource referrals to address client and family needs.
• Advocate for clients and families to navigate the community system.
• Document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.

🎯 Requirements

• Must be licensed by the NJ Board of Social Work Examiners at LMSW or LISW.
• MSW degree, with at least 2 years of community-based experience.
• Accreditation by the Council on Social Work Education (CSWE), demonstrated by transcript or degree.
• Strong interpersonal skills; ability to work independently.
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases, as assigned.
