Medical Social Worker, Home Health Visits

📋 Description

• Making home visits to clients in designated geographic territories.
• Assist team in understanding social and emotional factors in clients' health.
• Provide clinical social evaluations and plan interventions.
• Identify community resource referrals to meet client and family needs.
• Act as advocate for client and family to navigate care in the community.
• Participate in total care plan development with BAYADA clinicians.

🎯 Requirements

• Must be licensed by the NJ Board of Social Work Examiners at LMSW or LISW.
• Masters in Social Work with at least 2 years of community-based experience.
• Accreditation by the Council on Social Work Education (CSWE).
• Home care experience a plus.
• Ability to read, write, and communicate in English.
• Ability to work independently and manage time effectively.
