📋 Description

• Making home visits to clients in designated territories.
• Provide clinical social evaluations and plan interventions.
• Identify community resource referrals to address client and family needs.
• Act as advocate for client and family to navigate the system.
• Develop and maintain the plan of care with the multidisciplinary team.
• Document observations and interventions on a touch pad tablet.

🎯 Requirements

• Licensed LMSW or LISW in the state.
• MSW with at least 2 years of community-based exp.
• CSWE accreditation evidenced by transcript/degree.
• Strong English reading/writing and communication.
• Solid computer skills; EMR experience preferred.
• Excellent interpersonal skills.
