Medical Social Worker, Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Collaborate with a multidisciplinary team to provide skilled home health care.
• Provide clinical social evaluations and plan interventions based on findings.
• Identify community resource referrals to address practical and environmental needs.
• Advocate for clients and families to navigate the community system.
• Develop and maintain the total plan of care with BAYADA clinicians.

🎯 Requirements

• Licensed by the NJ Board of Social Work Examiners at LMSW or LISW.
• MSW with at least 2 years of community-based experience.
• CSWE accreditation demonstrated by transcript or degree.
• Home care experience a plus.
• Ability to read, write, and communicate in English.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases, as assigned.
