Medical Social Worker, Home Health

📋 Description

• Making home visits to clients in designated geographic territories.
• Assist team in understanding social and emotional factors related to health problems.
• Provide clinical social evaluations and plan interventions based on findings.
• Identify appropriate community resource referrals to address client and family needs.
• Act as an advocate for both client and family to navigate the patient through the community system.
• Participate in the development of the total plan of care with the multidisciplinary BAYADA team.

🎯 Requirements

• Must be licensed by the NJ Board of Social Work Examiners at the Master (LMSW) or Independent (LISW) level.
• A minimum of a Masters in Social Work, with at least 2 years of community-based experience.
• Accreditation by the Council on Social Work Education, as demonstrated by transcript or degree.
• Strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases, as assigned.
