📋 Description

• Make home visits to clients in designated geographic territories.
• Assess social and emotional factors related to health problems.
• Provide clinical social evaluations and plan interventions.
• Identify community resource referrals for client and family.
• Advocate for client and family through the community system.
• Document care observations and interventions in EMR on a tablet.

🎯 Requirements

• Master of Social Work (MSW) with at least 2 years of community-based experience.
• CSWE accreditation demonstrated by transcript or degree.
• Home care experience a plus.
• Ability to read, write, and communicate in English.
• Ability to work independently and manage time effectively.
• Solid computer skills; EMR experience preferred.
