Medical Social Worker, Home Health

📋 Description

• Making home visits to clients in designated geographic territories.
• Assist our team in understanding social and emotional factors in clients' health.
• Provide clinical social evaluations and plan interventions.
• Identify community resource referrals for patient and family needs.
• Advocate for client and family to navigate the community system.
• Participate in developing the total plan of care with the multidisciplinary team.
• Develop and maintain contracts with public/private agencies as resources.
• Educate patients and families about community resources to promote health.
• Document observations, interventions, and evaluations on a touch pad tablet.

🎯 Requirements

• Master’s in Social Work, with at least 2 years of community-based experience.
• CSWE accreditation shown by transcript or degree.
• Home care experience a plus.
• Read, write, and communicate in English.
• Ability to work independently and manage time.
• Strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
• Travel to cases, as assigned.
