📋 Description

• Making home visits to clients in designated territories.
• Assess social and emotional factors related to health.
• Provide clinical social evaluations and plan interventions.
• Identify community resource referrals to address client and family needs.
• Advocate for client and family to navigate the community system.
• Participate in the total plan of care with the multidisciplinary team.

🎯 Requirements

• Masters in Social Work with at least 2 years of community-based experience.
• CSWE accreditation documented by transcript/degree.
• Home care experience a plus.
• English communication skills (read/write).
• Ability to work independently and manage time.
• Strong interpersonal skills; EMR experience preferred.
