Medical Social Worker

📋 Description

• Making home visits to clients in designated geographic territories.
• Collaborate with a multidisciplinary BAYADA team to support home health care.
• Provide rehabilitative care planning and skilled nursing in the home.
• Document observations and interventions using a touch pad tablet.
• Advocate for clients and families to navigate the community system.
• Participate in development of the total plan of care with the team.

🎯 Requirements

• Must be licensed by the NJ Board of Social Work Examiners at the LMSW or LISW level.
• A Master’s in Social Work with at least 2 years of community-based experience.
• CSWE accreditation demonstrated by transcript or degree.
• Home care experience a plus.
• Strong English reading/writing and communication skills.
• Solid computer skills; EMR experience preferred.
