LPN - Transition Team

📋 Description

• Deliver quality home care to infants, children, and adults in Philadelphia.
• Manage transition of care from hospital or HICU to ongoing home care.
• Partner with CHOP to care for rare, complex diseases.
• Provide care and train BAYADA care teams.
• Overnight and day shift options.
• Supportive team environment with ongoing training.

🎯 Requirements

• Have a current Licensed Practical Nurse license.
• NICU, PICU, ICU, or home care trach/vent experience required.
• Pass high-tech Adult and Pediatric simulations.
• Hold current CPR certification.
• Ability to teach and educate BAYADA care team.
• Availability to commit to an ongoing, consistent schedule.
