LPN - Transition Team

📋 Description

• Provide direct bedside nursing and personal care for medically complex clients.
• Support safe transitions from hospital/rehab to home care.
• Deliver expert clinical interventions and ensure continuity of care.
• Train and mentor the care team to high standards.
• Travel throughout Ocean County to visit patients.

🎯 Requirements

• Current LPN license.
• Home care trach/vent, NICU/PICU/ICU experience required.
• CPR certification current.
• Ability to teach and educate the BAYADA care team.
• Availability to commit to an ongoing, consistent schedule.
• Ability to travel throughout Ocean County.
