LPN - Advanced Care Planning Specialist

📋 Description

• Conduct telephonic advance care planning conversations to identify and document patients' goals.
• Educate patients/families on interventions (feeding tubes, ventilation, CPR) in plain terms.
• Document conversations within 48 hours to generate state-specific advance care plans.
• Provide patient support via inbound calls, documentation review, and projects.
• Maintain knowledge of state laws on advance care planning via training.
• Other duties as assigned.

🎯 Requirements

• LPN with certificate or diploma in Practical Nursing.
• Current, unrestricted LPN license.
• 2+ years LPN or relevant healthcare experience.
• Knowledge of serious conditions (COPD, CHF, ESRD) and progression.
• Ability to adapt to shifting priorities in fast-paced environments.
• Familiar with Google Suite; able to learn call center and patient docs tech.
• Navigate multiple systems while speaking with patients.
• Strong verbal and written communication with active listening.
• Strong time management and attention to detail.
• Know state regulations on advance directives and Medicare/CMS guidelines.
