- Conduct telephonic advance care planning conversations to identify and document patients' goals.
- Educate patients/families on interventions (feeding tubes, ventilation, CPR) in plain terms.
- Document conversations within 48 hours to generate state-specific advance care plans.
- Provide patient support via inbound calls, documentation review, and projects.
- Maintain knowledge of state laws on advance care planning via training.
- Other duties as assigned.
π― Requirements
- LPN with certificate or diploma in Practical Nursing.
- Current, unrestricted LPN license.
- 2+ years LPN or relevant healthcare experience.
- Knowledge of serious conditions (COPD, CHF, ESRD) and progression.
- Ability to adapt to shifting priorities in fast-paced environments.
- Familiar with Google Suite; able to learn call center and patient docs tech.
- Navigate multiple systems while speaking with patients.
- Strong verbal and written communication with active listening.
- Strong time management and attention to detail.
- Know state regulations on advance directives and Medicare/CMS guidelines.
