- Perform and document wound care in clinic per PCP.
- Perform and document clinic procedures, including medication management.
- Administer medications and reinforce education to participants.
- Coordinate med order changes from same-day procedures with PCP.
- Review/reconcile medications during Welbe Welcome Day.
- Pre/post-operative touchpoints; follow instructions; escalate as needed.
- Flexibly adapt assignments as business needs evolve.
🎯 Requirements
- Graduate of Accredited School of Nursing with unencumbered LVN/LPN license
- Minimum of one year nursing experience with frail or elderly population
- Nursing knowledge to treat frail elderly patients and manage complex cases
- Motivated, self-directed; able to work in changing environments and emergencies
- Excellent clinical, organizational, and communication skills with seniors and teams
- Highly organized; able to manage multiple patients
- Must obtain CPR and First Aid within 180 days; certification on hire preferred
