- Perform and document wound care in clinic per scope of practice and PCP direction
- Perform and document procedures and care in clinic, including med management
- Administer ordered medications and reinforce education on meds
- Coordinate med order changes from same-day procedures; oversee changes
- Perform med review/reconciliation during Welbe Welcome Day
- Perform pre- and post-operative clinical touchpoints and follow-ups
🎯 Requirements
- Graduate of an Accredited School of Nursing with an unencumbered LVN/LPN License
- Minimum of one year nursing experience in clinical care with frail/elderly population
- Nursing knowledge and skills to treat frail elderly and manage complex clinical situations
- Highly motivated, self-directed, and able to adapt in a changing environment
- Excellent clinical, organizational, and communication skills with seniors and families
- CPR and First Aid certification within 180 days; certification at hire preferred
