📋 Description

• Making home visits to clients living in designated territories.
• Administering medication, wound care, treatments, and procedures.
• Monitoring clients' conditions; reporting changes to Clinical Manager.
• Following up with, executing, and documenting doctors' orders.
• Performing client assessments as necessary.
• Case management and coordination; EMR documentation on a tablet.

🎯 Requirements

• Active LPN license in NJ.
• Minimum one year of recent, verifiable LPN experience.
• Graduation from an accredited nursing program.
• Home care experience preferred, but not required.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills; solid computer skills.
