📋 Description

• Making home visits to clients in designated territories.
• Administering meds, wound care, treatments, and procedures.
• Monitor clients' conditions; report changes to Clinical Manager.
• Follow up on, execute, and document doctors' orders.
• Perform client assessments as needed.
• Case management and care coordination; document in EMR.

🎯 Requirements

• Current PA license as Licensed Practical Nurse (LPN).
• Minimum one year of recent verifiable LPN experience.
• Graduation from an accredited nursing program.
• Prior home care experience strongly preferred.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills; EMR experience preferred.
