Licensed Practical Nurse, (LPN) - Home Health

📋 Description

• Making home visits to clients in designated territories.
• Administer medications, wound care, treatments, and procedures.
• Monitor client conditions and report changes to Clinical Manager and Client Services Manager.
• Follow up with, execute, and document doctors' orders.
• Perform client assessments as necessary; case management and coordination.
• Document observations, interventions, and evaluations in EMR on a state-of-the-art touch pad tablet.

🎯 Requirements

• One year of LPN clinical experience required.
• Current North Carolina LPN license.
• Graduation from accredited nursing program.
• Prior home care experience preferred, not required.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills; solid computer skills with EMR.
