Licensed Practical Nurse, (LPN) - Home Health

📋 Description

• Making home visits to clients in designated territories.
• Administering medications, wound care, treatments, and procedures.
• Monitor client conditions and report changes to Clinical Manager.
• Follow up with, executing, and documenting doctors' orders.
• Perform client assessments as necessary.
• Case management and coordination.
• Document observations and care using EMR on a state-of-the-art touch pad tablet.

🎯 Requirements

• Current LPN license in North Carolina.
• Minimum 1 year LPN experience.
• Graduation from accredited nursing program.
• Prior home care experience preferred.
• Ability to work independently and manage time.
• Strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
• Ability to travel to assignments.
