Make home visits to clients in designated territories.
Administer medications, wound care, and procedures.
Monitor client conditions; report changes to Clinical Manager and Client Services Manager.
Follow up with, execute, and document doctors' orders.
Perform client assessments as necessary.
Case management and coordination; document in EMR on a tablet.

🎯 Requirements

Current SC LPN license.
At least one year of LPN experience.
Diploma or transcript from accredited nursing program.
Home care experience preferred.
Ability to work independently and manage time.
Strong interpersonal skills.
