Visit clients in their homes to help them recover from injury, illness, or surgery.
Case manage and develop care plans; provide assessments and wound care.
Progress clients from discharge to meeting goals at home.
Provide education to patients and families.

🎯 Requirements

Active and current LPN license.
One year of clinical experience preferred.
Home health care experience welcome, but not required; we will train you.
Ability to travel locally to cases as assigned.
