Making home visits to clients in designated territories.
Administering medications, wound care, treatments, and procedures.
Monitoring client conditions; reporting changes to Clinical Manager and Client Services Manager.
Following up with, executing, and properly documenting doctors' orders.
Performing client assessments as necessary.
Case management and coordination.

🎯 Requirements

Current NJ Licensed Practical Nurse license.
Minimum 1 year of recent, verifiable LPN experience.
Graduation from an accredited nursing program.
Ability to work independently and manage time effectively.
Strong interpersonal skills.
Solid computer skills; EMR experience preferred.
