Lead Community Health Worker

📋 Description

• Manage a caseload of patients with complex medical, behavioral, and social needs
• Complete assessments and co-develop care plans with interdisciplinary teams
• Build rapport to promote self-management, stability, and behavior change
• Train new CHWs; coordinate shadowing, onboarding, and skills
• Provide real-time coaching and mentorship through field visits
• Ensure accurate, timely documentation in case management systems

🎯 Requirements

• High School Diploma or equivalent required
• Valid driver's license, automobile insurance, and reliable transportation required
• 3+ years of CalAIM experience with homelessness, SMI/SUD, and complex cases
• Experience mentoring, training, or leading peers preferred
• Strong understanding of care coordination workflows and documentation standards preferred
• Ability to work independently in outdoor and community-based environments preferred
