Occupational Therapist - Home Modifications - Cincinnati, OH

Jukebox Health is looking for OTs who are passionate, empathetic, and committed to helping older adults and those with disabilities achieve their goal of living at home independently. 

• Complete home safety evaluations with clients and/or caregivers to determine ways to improve safety and independence in the home.
• Generate personalized reports for clients and/or caregivers, providing recommendations for basic home modifications and activity adjustments.
• Educate clients and caregivers on essential fall prevention strategies and techniques.
• Establish rapport and engage in collaborative discussions with clients and/or caregivers to meet their individual needs.

• State OT registration and licensure (required).
• Availability to conduct in-person home evaluations.
• Effective communication and clinical reasoning skills.
