Occupational Therapist

• Schedule: Must be available 20 hours a week, 3 days per week 
• Compensation: $500/ Day Rate

• Complete comprehensive, in-home safety evaluations and functional assessments with clients and/or caregivers
• Provide detailed, evidence-based recommendations for home modifications and activity adjustments
• Submit clinical documentation and reports through company documentation system in a timely manner
• Educate clients and caregivers on fall prevention, safe mobility, and independent living strategies
• Build rapport and collaborate with clients and caregivers to meet their unique goals
• Communicate with scheduling and operations team
• Communicate findings and insights with the internal clinical team as needed

• Current OT state licensure
• Minimum of 2+ years of occupational therapy experience
• Availability 3 days per week
• Background in home health, geriatrics, or disability services preferred
• Excellent communication, problem-solving and clinical reasoning skills
• Reliable transportation and valid driver’s license
• Comfortable with independent work and proficiency with documentation tools
• Ability to maintain team communication
