This position provides social work and bereavement support to patients and families.
Open to travel within the designated area.
Comprehensive psychosocial assessments.
Counseling and supportive interventions.
Advance Care Planning and decision support.
Bereavement support and follow-up.

🎯 Requirements

Master’s degree in Social Work (MSW) and Texas licensure.
Minimum of one year in social work, grief counseling, or bereavement in healthcare.
Clean driving record and valid driver's license and auto insurance.
Ability to travel within designated area.
Licensed Master Social Worker (LMSW) in Texas.
