📋 Description

  • Provide psychosocial support to patients and families.
  • Conduct comprehensive psychosocial assessments and care planning.

🎯 Requirements

  • Master’s degree in Social Work (MSW) and Indiana licensure.
  • Minimum 1 year of experience in social work, grief counseling, bereavement, or related healthcare experience (hospice/palliative care preferred).
  • Licensed Hospice Social Worker (or eligible) and strong counseling skills.
  • Clean driving record; valid driver’s license and auto insurance.
  • Willingness to travel within the designated area.
