Hospice, Physical Therapist

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measures of mobility, strength, and balance.
• Develop and implement individualized care plans with PT interventions (manual therapy, gait training, balance).
• Assess and revise care plans with nursing in the multidisciplinary team.
• Educate clients and families on rehabilitative care and safety.
• Document observations and interventions in EMR using a touchpad tablet.

🎯 Requirements

• A current New Jersey Physical Therapist - PT license.
• Minimum of one year of recent PT experience.
• Graduate of an APTA-approved program or AMA CAAHEA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
