Make home visits to clients in designated VT/NH territories.
Provide medication administration, wound care, treatments, and comfort care.
Monitor client conditions and report changes to the Clinical or Client Services Manager.
Follow doctors' orders and document appropriately.
Perform client assessments as needed.
Occasional on-call after hours and weekends.

🎯 Requirements

A current VT and NH RN license.
A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
Prior home care and/or hospice experience strongly preferred, but not required.
Solid computer skills; prior experience with EMR systems.
Ability to travel to cases as assigned.
