Follow designated care plan per patient needs
Make home visits to clients in designated territories
Administer medications; provide wound care, treatments and procedures
Monitor client conditions; report changes to Clinical or Client Services Manager
Follow up with, execute, and properly document doctors' orders
Perform client assessments and case management

🎯 Requirements

Current Massachusetts RN license
Graduation from an accredited nursing program
One year of recent clinical RN experience
Medicare Home Health / OASIS documentation experience preferred
Knowledge of home health care and patient safety standards
Ability to work in designated geographic territories
