Make visits to clients in their homes
Perform PT assessments: mobility, ROM, balance, ambulation
Develop and implement individualized care plans with manual exercises, gait training, balance
Continually assess and revise the PT plan with nursing in the multidisciplinary team
Educate clients and families in rehabilitative care and independence
Document observations and interventions using a touch pad tablet

Current MA Physical Therapist license
