Home Health Visits Registered Nurse

📋 Description

• Provide skilled nursing and rehabilitative care at clients' homes.
• Perform head-to-toe assessments and monitor subtle changes.
• Manage acute and post-acute care (CHF, COPD, wound care, IV therapy).
• Triage and independently manage urgent clinical situations at home.
• Conduct medication reconciliation, teaching, and safety monitoring.
• Perform wound assessment, care, and documentation.

🎯 Requirements

• Current nursing license in Massachusetts in good standing.
• Graduation from an accredited nursing program (transcript or diploma).
• At least one year of hands-on experience with adult/geriatric patients in acute or similar settings.
