📋 Description

• Make home visits to clients in designated territories.
• Assess mobility, ROM, gait, balance, strength, and safety.
• Implement wound care and protective interventions with evidence-based dressings.
• Develop and revise individualized PT care plans including manual therapy and gait training.
• Educate clients and families on rehabilitation and independent living.
• Document observations and interventions in the EMR.

🎯 Requirements

• Current PT license in good standing.
• Minimum 1 year clinical PT experience.
• Graduation from APTA-approved PT program or AMA-accredited.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficient computer skills; EMR experience preferred.
