Home Health Visits Physical Therapist

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests: mobility, gait, strength, balance, safety.
• Implement wound care and protective interventions with evidence-based dressings.
• Develop and revise individualized care plans, incl. manual therapy and gait training.
• Educate clients and families on rehabilitative care and safety.
• Document observations and interventions in EMR on a tablet.

🎯 Requirements

• A current MD Physical Therapist - PT license.
• Minimum of one year of recent PT experience.
• Graduation from APTA-approved program or CAAHEA-accredited program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
