📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests: mobility, ROM, transfers, balance.
• Implement protective wound interventions and dressings.
• Develop and implement care plans with manual therapy, gait, balance.
• Continuously assess and update PT care plans with nurses.
• Educate clients and families to promote health and independence.
• Document observations and interventions in EMR on a tablet.

🎯 Requirements

• Licensed Physical Therapist (PT) in good standing.
• Minimum 1 year PT experience.
• Graduate from an APTA/CAAHEA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, problem-solving skills.
• Solid computer skills; EMR experience preferred.
