Home Health Transitional Care Manager

📋 Description

• Build and strengthen relationships with referral sources and hospital contacts.
• Educate leaders about BAYADA services and home health care eligibility.
• Manage referral intake and coordinate initial transition to home care.
• Develop relationships with physicians and other health care professionals.
• Market BAYADA services by articulating features and benefits.
• Review medical docs to assess client eligibility and resources.

🎯 Requirements

• Current license in good standing as RN, LPN, MSW, PT or OT.
• Sales or marketing background is a plus.
• Strong interpersonal skills and professional demeanor.
• Knowledge of Medicare COPs and program requirements.
• At least one year of health care experience, home health preferred.
• Discharge planning experience is a plus.
