Conduct home health nursing assessments to determine equipment needs and goals
Actively participate in IDT meetings to develop participant care plans
Deliver and document home health nursing interventions per care plans
Instruct participants and families on disease process, self-care techniques, and prevention
Track and monitor home health scheduling with the Home Health Manager
Collaborate with physicians and IDT for smooth care transitions

🎯 Requirements

Graduate of an accredited school of nursing
Unencumbered RN license required
Minimum 1 year nursing experience with frail/elderly population; home health preferred
Reliable means of transportation
CPR and First Aid certification within 180 days of hire (preferred)
