Provide skilled nursing and rehabilitative care to home health clients per visit.
Work as part of a multi-disciplinary team to support care at home.
Travel to clients in your local area on a schedule that fits you.
Monitor clients' conditions and document changes.
Follow doctors' orders; perform client assessments and care coordination.
Per diem/part-time schedule; orientation for new home health hires.

🎯 Requirements

One year RN experience required.
Colorado RN license in good standing.
Home Health and OASIS experience preferred.
If no Home Health experience, orientation 3 days/week for 4+ weeks.
Graduation from an accredited nursing program.
