Conduct home health nursing assessments to determine equipment needs and goals
Actively participate in IDT meetings to develop participant care plans
Deliver and document nursing interventions per care plans, incl. meds/tests
Instruct participants and caregivers on disease processes, self-care, prevention
Track and coordinate home health scheduling with the Home Health Manager
Collaborate with physician and IDT for smooth care transitions

🎯 Requirements

Graduate of an accredited school of nursing
Unencumbered RN license
Minimum 1 year nursing experience with frail/elderly; home health preferred
Reliable transportation
CPR and First Aid within 180 days of hire (preferred at hire)
