- Deliver and document nursing interventions per care plans, incl. meds/tests
- Instruct participants and caregivers on disease processes, self-care, prevention
- Track and coordinate home health scheduling with the Home Health Manager
- Collaborate with physician and IDT for smooth care transitions
π― Requirements
- Graduate of an accredited school of nursing
- Unencumbered RN license
- Minimum 1 year nursing experience with frail/elderly; home health preferred
- Reliable transportation
- CPR and First Aid within 180 days of hire (preferred at hire)
