- Conduct home health nursing assessments to determine equipment needs and goals
- Actively participate in IDT meetings to develop participant care plans
- Deliver and document nursing interventions per care plans, including meds and tests
- Instruct participants, families, and caregivers on disease process, self-care and prevention
- Track and monitor home health scheduling with the Home Health Manager or scheduler
- Collaborate with physicians and IDT for smooth care transitions between settings
π― Requirements
- Graduate of an accredited school of nursing
- Unencumbered RN License required
- Minimum one year nursing experience with frail/elderly population; home health preferred
- Reliable means of transportation
- Must obtain CPR and First Aid within 180 days of hire; maintain certification thereafter
- Ability to work in participants' homes and travel as needed
