Home Health RN

📋 Description

• Conduct home health nursing assessments to determine equipment needs.
• Participate in IDT meetings to develop care plans.
• Deliver and document nursing interventions per care plans.
• Instruct participants and families on disease, self-care, and prevention.
• Track and monitor scheduling with the Home Health Manager.
• Collaborate with physicians and IDT for smooth care transitions.

🎯 Requirements

• Graduate of an accredited school of nursing.
• Unencumbered RN license required.
• Minimum 1 year nursing experience with frail/elderly; HH preferred.
• Reliable means of transportation.
• CPR and First Aid cert within 180 days; maintain thereafter.
