📋 Description

• Conduct initial assessments to determine care needs and plan care.

🎯 Requirements

• BSN with NYS license or Associate degree with homecare experience.
• 1-2 years nursing experience in homecare.
• Geriatric care or disability experience.
• Ability to travel within the five boroughs.
• Knowledge of state and federal home health regulations.
• Strong interpersonal, verbal and written communication skills.
