Home Health Physical Therapist (PRN)

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility, ROM, strength, balance.
• Implement wound prevention and care using evidence-based dressings.
• Develop and implement individualized care plans, including manual therapy, gait training, balance.
• Continually assess and revise PT care plans with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care to promote independence.

🎯 Requirements

• VA Physical Therapist license.
• Minimum of one year of PT experience.
• Graduate from an APTA-approved program (or AMA/CAAHEA accreditation).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
