Perform mobility and ROM tests, transfer status, balance, and safety assessments.
Develop and implement care plans including manual therapy, gait training, and balance interventions.
Continuously assess and revise PT care plan; collaborate with nursing in the care team.
Educate clients and families on rehabilitative care to promote health, safety and independence.
Accurately document observations and interventions using a touch pad tablet.

🎯 Requirements

Current Colorado Physical Therapist license.
Experience with Medicare Home Health and OASIS preferred.
